DPKO/DFS is using non-punitive aviation safety reporting system, which includes occurrence and hazard reports.


Reporting of occurrences and hazards is mandatory for all aviation related personnel – they must report all occurrences and observed hazards to Mission Aviation Safety staff.


All other Mission personnel are encouraged to report any situation or condition related to aviation operations, which in their opinion is hazardous to the safety of flight or endangers people’s life. This type of report is voluntary, and can be submitted anonymously, so that the identity of the reporter is not compromised.


Authorized Mission personnel after being assigned with their specific roles should electronically compile the reports. The system replicates reports via the DPKO/DFS communications channels, so that reports become ‘visible’ almost in real time at UNHQ and in the other Missions.


Aviation Safety Section, LSD/DFS is responsible for collating and analysis of safety information received from the Missions and further dissemination of information to the concerned staff and managers.


OCCURRENCE AND HAZARD REPORTING

Occurrence Report (OR) is intended to provide information on the circumstances of any accident or incident involving the DFS-contracted aircraft. Normally, the OR should be compiled in the reporting database within 24 hours.


Hazard Report (HR) is a tool that allows everyone in the Mission to report actual or potential hazards or hazardous conditions identified in various areas, thus contributing to the enhancement of aviation safety in the Mission. Aviation Safety staff should use all means available to make all Mission personnel aware of the importance of hazard reporting.
